Information and Referral Services

Resources and services available for persons with disabilities and/or their families.

Independent Living Skills Training

Budgeting, personal grooming, interpersonal communication, problem solving, cooking, home maintenance and other skills necessary to live independently.

Peer Counseling Services

Professional support, advice and assistance in resolving personal issues specific to a disability.

Individual and Community Advocacy

Advocacy is provided to remove both attitudinal and physical barriers in the community and to foster the belief that all individuals have a right to live independently in the community of their choice.

Training and assistance is provided to individuals so they may successfully advocate for their personal rights.

Accessibility surveys and suggested modification services are available for businesses, service organizations and the general public.

Vocational Services

Education, training and referral services are provided to assist the individual in obtaining their employment goal.

Community Living Assistance

Personal care assistance services to avoid nursing home placement, assistance in moving back into the community and training for individuals so they may successfully hire, train and direct their personal attendants.

Technical Assistance

Provides assistance with modifications to make home or work accessible in order to carry out daily activities.

Housing Services

Assistance and training is provided to individuals in locating, securing and maintaining adequate housing. Help to determine accessible housing modifications is also available by an accessibility specialist.

Home and Community Based Services (HCBS)

Three Rivers provides eligibility screening for the following Medicaid Waiver Programs.

  • Physical Disability Waiver
  • Traumatic Brain Injury Waiver

Case management services can be provided once waiver services are approved.

Payroll Services

Provides payroll services for individuals who self-direct their care under the following Waivers/Programs:

  • Physical Disability Waiver
  • Traumatic Brain Injury Waiver
  • Frail Elderly Waiver
  • MR/DD Waiver
  • Technical Assistance Waiver
  • Income Eligible Programs
  • Private Pay Programs
