Services for individuals of all ages with all types of disabilities
Provides services to qualifying individuals between the ages of 16 and 64 who
have a physical disability, receive Medicaid and SSI or SSDI. Includes Personal
Attendant Care, Assistive Services and Independent Living Counseling.
Provides services to qualifying individuals between the ages of 16 and 65 who
have a traumatic brain injury. Services provided include Skills Training,
Independent Living Counseling; Physical, Occupational, Speech, Cognitive and
Psychiatric Therapy.
Provides services to assist individuals in living in the community of their choosing and ensuring independent living.

If so, our HOME program may be exactly what you are looking for. Case management, Companion Care and Homemaker Services are available at reasonable rates to persons who are elderly or have a disability and seek assistance in their homes. Our dedicated staff provides quality, professional services. You have the convenience of making one payment each month for services provided. We take care of the details such as worker's compensation, applicable taxes and unemployment insurance. You maintain peace of mind knowing that your needs and safety are met in the comfort of your home.
3Rivers provides a unique opportunity for individuals to recycle useful medical equipment. We receive donated items and make them available to the general public at no charge. A variety of medical equipment items are available for loan. These items are loaned at no charge to anyone in temporary need of such items. Items may include, but are not limited to the following: walkers, canes, wheelchairs, hospital beds, shower chairs and more.
Items are provided as is, and 3Rivers maintains no responsibility for the use, maintenance and functioning of the donated medical equipment.
